      No owner means no autonomous execution. Record the authorized person or agent and the scope of permission.

      Preserve required human approval before marking an action completed. Authorization is not inferred from a score.

      Record the action time, changed system and resulting source record. Distinguish an attempted change from a completed one.

    4. 04

      Retain completed, reversed and escalated states. Reversal is evidence, not a reason to erase history.
